Diagnostic Code 6204Ear, Hearing & BalanceDiseases of the Ear38 CFR 4.87

Peripheral Vestibular Disorder

Peripheral vestibular disorders are inner-ear balance problems causing dizziness and unsteadiness (including labyrinthitis, vestibular neuritis, and BPPV). VA rates them at 10 or 30 percent based on dizziness and staggering.

How the VA rates it

Rated under diagnostic code 6204 with two tiers: 10 percent for occasional dizziness, and 30 percent for dizziness with occasional staggering. A compensable rating requires objective medical findings supporting a diagnosis of vestibular disequilibrium — subjective dizziness alone is not enough. Unlike Meniere's disease, any accompanying hearing loss is rated separately and combined.

Secondary condition strategy

This is often secondary to

If you already have one of these service-connected, it may support connecting this condition.

  • Traumatic Brain InjuryVestibular dysfunction is a common residual of TBI and blast exposure and is frequently claimed secondary to it.
  • Ear infections / otitis mediaChronic middle or inner ear infections in service can lead to lasting vestibular damage.

Evidence that strengthens this claim

  • Objective findings are mandatory for a compensable rating — get vestibular testing (VNG/ENG, rotary chair, Dix-Hallpike) documenting the disequilibrium, not just reported dizziness.
  • Document staggering or falls in medical records and lay statements; 'occasional staggering' is the difference between 10 and 30 percent.
  • If your symptoms include episodic vertigo with hearing loss and tinnitus, ask whether Meniere's disease (code 6205, up to 100 percent) is the more accurate diagnosis.

Rating rules to know

Hearing impairment or suppuration occurring with a peripheral vestibular disorder is rated separately and combined. But if the vertigo is part of Meniere's disease rated under 6205, it cannot also be rated under 6204.
